Sample ADA Statements for Course Syllabi

A good way to facilitate open communication with your students is to include an ADA statement on your syllabus. Please feel free to use these statements as you see fit.

Sample 1. If you are a student with a documented disability who will require academic accommodations in this course, please make an appointment with The Coordinator of Disability Services, in the library Room 504 on the 5th floor of the library or call  ext. 4546.
