Complete this form to request approval for courses taken at another institution. Please note that the maximum number of semester hours a student may transfer is 60. Juniors and Seniors who seek to transfer more than 8 semester hours from a community college must petition the Committee on Academic Standards. If you are within the last year of residence (30 semester hours) you must petition the Committee on Academic Standards for a waiver of the residency requirement. - Request for Experiential Learning Credit (pdf)

Students are responsible for gathering all materials necessary to support their petition to the Committee on Academic Standards.

Use this form to petition for Spring or Fall Term late add/registration, overload, or other reason (waiver of residency requirement, transfer of courses not pre-approved, etc.). - Grade Change Request (pdf)
For Faculty and Instructors ONLY! - Late Withdrawal from a Course (pdf)
Late withdrawals are possible only by petition to the Committee on Academic Standards. Such petitions are generally approved only when there are unforeseen circumstances occurring after the deadline which prevent a student from completing a course. A low or failing grade in a course is not sufficient reason for the committee to approve a late withdrawal. - Overload (pdf)
