New Student Forms
New first-year students must complete the following forms by the dates indicated below. Please note that username and password information, needed for some forms, is sent upon enrollment.
REQUIRED FORMS FALL 2014
- Housing & Roommate Preference Questionnaire (online submission)

- Commuter Application Form (online submissions - commuter students only)
- Financial Responsibility Form (pdf—print, complete & mail, scan to email@example.com or fax 607-431-4006)
- Three-Year Bachelor's Degree Enrollment Form and Deposit (if applicable)
DUE JULY 15:
- Medical Forms (pdf—print, complete, & mail): Note that a health professional appointment may be needed.
- Final, official high-school transcripts must be sent to:

- Health Insurance Election Form (online submission - will be available in early summer)
- Family Information Form (online submission)
