New Student Forms for Spring Term Enrollment

New students enrolled for Spring Term (beginning in February) must complete the required forms below by January 31. (Note: students enrolled for Fall Term will begin completing new student forms in April).

Commuter Application Form (online submissions - commuter students only)

Financial Responsibility Form (pdf-print, complete & mail, scan to studentaccts@hartwick.edu or fax 607-431-4006)

Medical Forms  (pdf-print, complete, & mail): Note that a health professional appointment may be needed. Although the form states that it is due July 15, please remember that you must complete this form prior to starting classes in February.

Final, official high-school transcripts must be sent to:
