Protocol for Hartwick College Student Athletes
1. New student athletes, freshmen/transfer students, must have completed new student medical examination forms on file at Perrella Wellness Center.
2. Upper-class students new to sports or who skipped a season must have a sports physical done at Perrella Wellness Center or by their health care provider within the past 3 months. Download necessary form below.
3. Returning athletes must have a sports physical done every summer by their health care provider. Download necessary form below.
All sports physical forms are required to be on file at Perrella Wellness Center at least one week prior to sports participation.
If forms are not on file at Perrella Wellness Center, an athlete will not be permitted to participate in team sports. There will be no exceptions.
