Housing Accommodations

Students seeking housing accommodations should follow this procedure:

1. Complete the Housing Voluntary Disclosure of a Disability form.
2. Provide proper documentation from a licensed Health Care Provider. The provider may complete this Medical Provider Form or write a letter to indicate the necessity of the housing request.
3. Students may be required to meet directly with a Hartwick medical or mental health provider in the Perrella Health Center.
4. Students requesting an accommodation for a dietary need, must meet with the Director of Dining Services or designee to discuss needs, and options available within the dining services to meet these needs.

All information will be considered confidential and will be released only to appropriate personnel on a need-to know basis.

The Housing Accommodations team (staff from AccessAbility Service, Residential Life and Housing, Perrella Health and Counseling Centers) will review the request and documentation and will provide notification of approval or denial. The determination of the housing accommodations team may include an accommodation that addresses what is medically necessary and may not be specifically the accommodation requested. Supporting documentation from a medical provider does not guarantee that the request for an accommodation will be supported as medically necessary.

The review process may take up to two weeks from the time all final documents are received by the accommodations team.
It is recommended that students start this process prior to arrival on campus.
