Hartwick has articulation agreements with The Sage Colleges of Health Sciences for graduate study in occupational therapy and physical therapy.

Both programs agreements are outlined below.

Occupational Therapy (MS OT)

Articulation Agreement with The Sage Colleges School of Health Sciences Occupational Therapy Program:

Hartwick College and The Sage Colleges of Health Sciences have developed a collaborative agreement to provide qualified students from Hartwick College who wish to pursue an MS in Occupational Therapy (OT) from the Sage Graduate Schools (SGS) with preferred admission as indicated below. This collaborative program provides for:

• Preferred acceptance into the Sage Graduate Schools for students who have completed occupational therapy program course prerequisites and with completed application;
• Waiver of the Sage Graduate Schools application fee;
• Early acceptance;
• Access to Sage advisors to coordinate undergraduate electives and coursework to reduce or eliminate prerequisites in the graduate program.

Terms of the Agreement: Occupational Therapy Admission

Hartwick College students will receive preferred admission to the Sage School of Health Sciences for the MS in OT degree, who demonstrate the following:

• earned baccalaureate degree;
• minimum overall 3.25 GPA that includes completion of occupational therapy program prerequisite courses with grades of C or better (see prerequisite courses listed below) and a 3.0 prerequisite science GPA (Anatomy and Physiology I and II, Physics I).

Requirements for Placement in the Sage Graduate School OT Program:

To be given priority in placement in the OT program students agree to submit the following information:

By June 1 of the year preceding graduation from Hartwick College:

1. Complete application online at: https://www.sage.edu/admission/graduate/

• The career goals essay & resume can be submitted with the online application.
• The supplemental forms can be emailed as attachments to [email protected] this includes:
clinical observation form, program prerequisite record, OT program academic standards
statement.

2. Email the OT Department, [email protected], and Sage Graduate School Admissions, [email protected], indicating application has been submitted via Articulation Agreement with indicated school.

3. Submit official transcripts from all college institutions attended.

4. Submit one letter of reference from the Hartwick College faculty and an additional
recommendation letter.

Within 30 Days of acceptance:

5. Make enrollment deposit of $300 (non-refundable, though to be applied toward graduate tuition)

Upon graduation from Hartwick College

6. Submit official transcripts documenting completion of undergraduate degree and OT Program
prerequisite courses

7. Submit Clinical Observation Document: 20 hours of clinical observation with an occupational
therapist

Physical Therapy (DPT)

Articulation Agreement between Hartwick College and The Sage Colleges School of Health Sciences Doctor of Physical Therapy Program:

Hartwick College and The Sage Colleges School of Health Sciences have developed a collaborative agreement to provide qualified students from Hartwick College who wish to pursue a Doctor of Physical Therapy (DPT) from the Sage Graduate Schools (SGS) preferred admission as indicated below. The collaborative agreement provides for:

• Preferred acceptance (on a space available basis) into the Sage Graduate Schools for students who have completed physical therapy program course prerequisites, after initial advisement and interview, and with completed application;
• Waiver of the Sage Graduate Schools application fee;
• Early acceptance;
• Access to Sage advisors to coordinate undergraduate electives and coursework that satisfy the prerequisites in the graduate program.

Hartwick College students will be given preferred admission to Sage Graduate School of Health Sciences for the DPT degree, who demonstrate the following:

• earned baccalaureate degree (or in progress)
• minimum overall 3.25 GPA;
• minimum science GPA of 3.25 (Anatomy & Physiology I and II; Physics I & II with labs or approved equivalents; Physics I & II with labs; Chemistry I & II with labs, and Cell Biology with labs);
• completion of Physical Therapy Program prerequisite courses with grades of C or better.

Requirements for Placement in the Sage Graduate School PT Program:

To qualify for preferred admission to the DPT Program students agree to the following:

By August 15* of the year preceding graduation from Hartwick College:

1. Submit a completed PTCSA Applications (www.ptcas.org) following the Early Decision Procedures, including:
   o Official Transcripts
   o Two (2) letters of recommendation, one of which must be academic
   o Clinical Observation Hours Documentation (minimum of 40 hours, preferable in at least two settings)
   o Resume
   o Essay
2. Submit the supplemental document from (DPT Academic and Technical Programs Standards Statement) to the PT Department. Forms can be found online.
   o Mail To: PT Department

3. Email the PT Department, [email protected] indicating that their application has been submitted to PTCAS via Articulation Agreement with Hartwick College.

Within 30 Days of acceptance:

1. Submit official transcripts from all college institutions attended to the PT Department at the address above.
2. Make enrollment deposit of $300 (non-refundable, though to be applied toward graduate tuition).
3. Upon graduation from Hartwick College:
4. Submit final transcripts documenting completion of undergraduate degree and PT Program prerequisite courses to the PT Department at the above address.
